Book a Free ConsultationInternational nurses applying to work in the UK NHS need to meet English language requirements at B2 level. In most cases this means passing a B2 SELT (ISE II from Trinity College London) covering all four skills — Speaking, Listening, Reading and Writing.
International nurses applying for NMC registration and a Health and Care Worker visa need to demonstrate B2 English proficiency. There are two main routes:
A Secure English Language Test at B2 level — accepted for both the Health and Care Worker visa and NMC registration (under the alternative route).
The NMC also accepts IELTS Academic at an overall Band 7.0 (with no skill below 6.5) or OET at Grade B in all four skills.
At B2 level, you can understand complex texts and instructions, communicate fluently with colleagues and patients, write clear professional documentation, and handle clinical conversations with confidence.
For nurses, B2 English is not just a visa requirement — it is a patient safety standard. Being able to take an accurate patient history, explain a procedure clearly or communicate a handover accurately all require B2-level English.
A structured speaking assessment covering factual topics, personal opinion and discussion with a trained examiner.
Understanding spoken English at natural speed across a range of topics and accents.
Reading comprehension tasks on factual and analytical texts at B2 level.
Extended formal writing tasks at B2 level — the section many nursing candidates find most challenging.
The B2 ISE II is more demanding than many nurses expect — particularly the Writing component, which requires structured formal writing rather than the clinical notes or handover records you may be more familiar with.
